Meaning
based on, using … as material or a starting point
〜をもとに means "using X as the basis or material": 実話をもとに映画が作られた (a movie made from a true story). X is a source that is transformed into something new: a story, data, an idea, experience. It differs from に基づいて, which frames X as a rule or standard to follow.
Both speech and writing, neutral register. Very common in descriptions of creative works, product development, and reports (アンケートをもとに). The noun-modifying form 〜をもとにした + N is frequent.
