Meaning
only at / only for …; being …; at once … and …
〜にして attaches to a noun and has three main jobs. (1) "Only at that stage / even for that person": 三十歳にして (only at thirty), 専門家にして初めて (only an expert). (2) "Being X and also Y": 科学者にして詩人 (a scientist and at the same time a poet). (3) Set phrases: 一瞬にして (in an instant), 幸いにして (fortunately), 生まれながらにして (from birth).
Literary and formal written Japanese: biographies, essays, speeches. Sense 1 stresses that something happened late, or only a special person could manage it; sense 2 is a formal way to list two qualities of one person. The set phrases in sense 3 also appear in speech.

Sense 1 with a person: only an expert, and even for an expert it takes effort (初めて).
彼は四十歳にして、ようやく自分の店を持った。
かれはよんじゅっさいにして、ようやくじぶんのみせをもった。
At forty, he finally got his own shop.
Sense 1 with an age: 四十歳にしてようやく = "not until forty". The age is late for the achievement.
彼女は科学者にして詩人でもある。
かのじょはかがくしゃにしてしじんでもある。
She is a scientist and at the same time a poet.
Sense 2: two roles joined. Often followed by でもある.
火事で、長年集めた本が一瞬にして灰になった。
かじで、ながねんあつめたほんがいっしゅんにしてはいになった。
In the fire, the books I had collected over many years turned to ash in an instant.
一瞬にして is a set phrase meaning "in an instant".
あの名人にして、この問題は解けなかった。
あのめいじんにして、このもんだいはとけなかった。

Person + にして + negative = "even X could not". Compare 〜でさえ.
Similar grammar and the difference
"In the situation of" (非常時にあって); marks circumstances, not an age or a special person. Compare side by side
生まれながらに is close to 生まれながらにして; ながらに means "in the state of" and is not used with ages. Compare side by side
"For a …" (contrary to expectation). Same start, different meaning: 子供にしては上手だ judges against a standard. Compare side by side

Sense 1 has two flavours: an unexpectedly early or late age (五歳にして・六十歳にして), and "even X" for a person of high ability (名人にして解けない). In sense 2 the two nouns are two faces of one person, often admired: 実業家にして慈善家. 今にして思えば ("looking back now") is a common set phrase in memoirs and speeches.

On the exam
- If the blank follows an age and is followed by 初めて / ようやく / やっと, choose にして.
- にしては is the standard distractor. Check whether there is an evaluation against a standard (にしては) or a stage / person being stressed (にして).
- Set phrases such as 一瞬にして and 幸いにして appear in 文法形式の判断 as single chunks; memorise them.
