The difference

〜ことにする "decide to": 〜ことにした announces a decision as final. 〜ようと思う keeps it as an intention.

〜ようと思う / 〜ようと思っている "I think I will / I intend to": ようと思う is an intention still forming ("I think I'll"). ことにした is a decision already made.
